Assumption of Risk and Agreement to Hold Harmless

I hereby agree to hold Canine University, the trainers and assistants, relations thereto, sponsors of the training, owners of the premises upon which the training is being held and their employees and guests, harmless from any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of this dog while in or upon the training premises or grounds or near any entrance thereto, and I personally assume all responsibility and liability for any such claim; and I further agree to hold the aforementioned parties harmless from any claim for loss of this dog by disappearance, theft, damage or injury, be caused or alleged to be caused by the negligence of the aforementioned parties, or by the negligence of any other person, or any other cause or causes. I assume the sole responsibility for and agree to indemnify and save the aforementioned parties harmless from any loss and expense (including legal fees) by reason of the liability imposed by law upon any of the aforementioned parties for damage because of bodily injuries, including death at any time resulting therefrom, sustained by any person or persons, including myself or on account of damage to property, arising out of or in consequence of my participation in this training, howsoever such injuries, death or damage to property may be caused, and whether or not the same may have been caused or may have been alleged to have been caused by the negligence of the aforementioned parties or any of their employees or agents, or any other persons.

I agree that the Canine University or the trainer has the right to refuse this registration or excuse the dog from class for cause which they shall deem to be sufficient.

Furthermore, my inserting my name below, I acknowledge and accept the foregoing paragraphs and agree to follow Canine University Policy & Procedures.

Tuition is due with the registration in order to hold your seat in the class. (This form is not currently submittable online.) You can also cut and paste into an email.

CLASS CONFIRMATION: You will receive a confirmation card or call prior to Lesson 1. Call us if you do not receive either.

Dogs who bark excessively as well as dogs who show signs of being aggressive and are potentially dangerous to people or other dogs will not be permitted in classes.

Withdrawal: Class tuition is non-refundable once the class has begun.
