CGC/Therapy Dog International Testing

Complete and print this form. U.S. Mail the form with $10 testing fee (payable to Canine University) to Canine University, 24850 Aurora Road, Bedford Heights, OH 44146. If you are also taking the Canine Good Citizen test, then an additional $10.  Fee is due with the registration in order to hold your time slot.

Mail completed registration and $10 testing fee payable to Canine University, to 24850 Aurora Road, Bedford Heights, Oh 44146.  If you are also taking the Canine Good Citizen test, then an additional $10.

Withdrawal: TDI fee is non-refundable one week prior to testing.

Important

*  You need to bring your dog's current vaccination record, rabies certificate and your dog's county dog license.

*  Only flat buckle collars are permitted for testing.

*  Come 10 minutes ahead of your scheduled time to check in.

*  Dogs must be at least one year of age on the test date.
